DEED OF WAIVER OF RIGHTS AND INTERESTS

KNOW ALL MEN BY THESE PRESENTS:

I,__________________________________, of legal age and with permanent address

at_________________________________________, after having been duly sworn to in
accordance with law, do hereby depose and say that:

I am the creditor of ____________________________, for the total amount of P150, 000

with interest amounting to P15, 000 payable every 1st, 15th and 24th of every month divided
evenly. That for the consideration that the debtor has been a good payor for a number of years, I
as creditor do hereby state that I am WAIVING all interests with regard to payments to be made
every 24th of the month amounting to P5, 000. Provided that the debtor pays on time every 1 st,
15th and 24th of the month, any and all payments made on the 24 th would be directly deducted to
the principal amount of P50,000. This waiver shall immediately be revoked if the debtor fails to
make any payments on time.
