SUBJECT : Notice of Written Explanation

The instant complaint was indorsed to the undersigned Investigator

for proper investigation and recommendation for a possible administrative
sanction/liability.

Relative thereto, in the interest of fair play and objectivity, you are
hereby directed to submit your written explanation before the
undersigned Investigator within forty eight (48) hours upon receipt
hereof as to why you should not be held accountable for the said
allegations. Attached is the copy of the Complaint Affidavit of Mr. William
Estoce together with annexes dated 10 October 2019 for your reference.

Failure to submit the written explanation shall be construed as a

waiver on your right to be heard and shall compel the undersigned to
proceed with the appropriate administrative actions against you in
accordance with Civil Service rules and regulations.
