1. Describe the work for which the candidate was responsible for:

He was a power plant operator who was assigned to operate, monitor, troubleshoot and
improve efficiency of the operation of several Diesel-fired generator set and its auxiliaries.

2. How would you describe the candidate’s relationship with superiors, co-workers,
and subordinates?

His relationship with all of us was really good, he is a hardworking, obedient and respectful
to all of us that includes supervisors, co-operators, security guards, OJT’s and janitors.

3. Did the candidate have a positive/negative work attitude? What specific instance
did such attitude manifest?

I haven’t seen a problem with his attitude. He is professional and a friendly to


everyone.

4. How would you describe the quality and quantity of output generated by the
employee?

He was a performer for a several of years in our company. That says it all.

5. What were his strengths?

He was very good in making report, and because of that he was always be the one
appointed by our supervisors in terms of reporting in PowerPoint. He always get the job
given to him ahead of time. He trustworthy and loyal.

6. What were his weaknesses?

Not to be bias, I can’t think of one. Maybe room for improvement in speaking in
front of a crowd.

7. How was the candidate’s attendance record?

I think he has excellent attendance record and got a perfect attendance award.

8. Were there any infractions/negative incidents committed and complaints filed


against the employee?

None.

9. What is your overall assessment of the candidate?


He is a positive minded kind of person, that will really help any company that his
working for. He’s a really great person.

10. Would you recommend his for the position? Why? Why not?

Absolutely yes, he possess a combination of skills and experience that make him
stand out from the crowd. He can do the work and deliver exceptional results.
