HAZARDS & EFFECTS MANAGEMENT PROCESS

1.0 Introduction

This document is prepared in accordance with the HSE Systems Procedure for
Hazards & Effects Management, HSESP-005, to ensure that the hazards
involved in various activities carried out by the Company are identified, their
risks assessed, necessary controls are implemented and plans are in place for
recovery, in case the controls fail.

This will be used as guidelines and shall be disseminated to employees through


toolbox talks for implementation along with any new hazards and controls
identified with reference to site conditions based on the assessment / review by
the Site Engineers / Supervisors prior to commencement of any activity.

2.0 Hazards & Effects Management

The following are the steps involved in Hazard & Effects Management
Process:

 Identify the Hazards and its Effects.


 Eliminate the Hazard, if possible.
 Evaluate the Risks.
 Determine the Controls in Place and its Effectiveness.
 Reduce the Risk by Applying Additional Controls.
 Assess Residual Risk, if any, after Controls are in Place.
 Evaluate the Contingency / Recovery Plans.

3.0 Risk Assessment

Risks associated with the hazards are assessed based on the Risk Assessment
Matrix (RAM), shown below:

4.0 Hazard Control

The following control measures generally apply to all hazards that shall be
adopted as a proactive measure in addition to the specific controls identified in
the HEMP Register of the activity concerned:

 Strict adherence to Customer & Company HSE requirements.


 Compliance with Safety Data Sheets / SHOC Card, for use of chemicals.
 Permit-to-work, where applicable
 Use of recommended tools and equipment.
 Deploying right / competent person for the job.
 Training and instructions related to the job.
 Conduct of toolbox talks prior to work commencement
 Good Housekeeping.
 Provision of adequate warning signs, barricading, etc.
 Adequate and effective supervision.
 Periodic inspections
 Use of necessary PPE (refer section below).

5.0 PPE Requirements

 Helmet, Safety boots, Goggles & Coverall – for all employees.


 Hand Gloves – for manual handling of materials.
 Face Mask, Ear Muff, Aprons, Gumboots, reflective vests, and other trade
specific PPE – to be provided as identified in the HEMP Register w.r.t to
the hazards involved.

5.1 Responsibility

5.1.1 Project / Construction Manager shall:

 Assess the demand and arrange for procurement of PPE’s


 Ensure recorded issue of PPE’s as per job specific requirements.

5.2.2 Site Engineers / Supervisors shall:

 Enforce the use & ensure that the employees wear the prescribed PPE’s and
maintain it properly.

5.2.3 HSE Advisor shall:

 Advise Project / Construction Manager on the quality and type of PPE’s


required
 Educate the workers & monitor the use and care of PPE’s.

6.0 Recovery Measures / Emergency Response

Site specific emergency response plans shall be in place to reduce the gravity
of consequence and to regain control over the situation within the least possible
time, for any foreseeable incident.

The following shall be the action in such contingencies:

 Look at the situation quickly and carefully.


 Assess what kind of help is required.
 Call for help, seeking assistance / resources to handle the situation

(Site Supervisor shall during toolbox talks ensure that all employees have
ready access to telephones for use during emergencies and that they are
familiar with the number (s) to be contacted in addition to the number of the
Supervisor).

 Respond to the emergency (provide first aid / fight the fire / contain spill,
etc.) if you are trained and / or wait / follow-up for further assistance.
 Remove the casualty from any danger, only if remaining there is life
threatening, as the movement may worsen the injury, provided your safety
is not at stake.
 Isolate the hazard that caused the incident to avoid further aggravation /
recurrence.
 Preserve the area / incident scene to facilitate investigation.
 Mass meeting with employees to disseminate the immediate learning
available.
 Resume work on getting clearance from the Project Manager.
 Incident Reporting & Investigation
 Dissemination of causes, learning points and actions required to prevent
recurrence.

7.0 HEMP Review & Feedback

Users of this document who encounters any mistake, inappropriate or


confusing entry, inadequacies, any new hazards / controls identified and HEMP
for any new activities are requested to notify QHSE Systems Unit through User
Feedback Form in Management Systems Procedure for Document & Data
Control, MSP-001, to render this document live and up-to-date.
