- Appointment as the Employee Representative of Joint Safety and Health

Committee

Please be informed that with immediate effect, you are hereby appointed as the Employee
Representative of Joint Safety and Health Committee for the project 132kV Kuchai Lama
GIS Switching Station.

You are hereby required to carry out duties and responsibilities as stipulated in the
Occupational Safety and Health Act 1994 and the regulations as per below:-

1. Assist in the development of safety and health rules and safe system of work such as
SAFCA, SEMS and OHSAS 18001:2007.
2. To conduct and ensure Joint Safety and Health Committee meeting is held once in
every 3 months.
3. To ensure Site Safety and Health Committee meeting is held every month.
4. To comply with the Company's Policy on Occupational Safety and Health.
5. To comply with OSHA 1994 and other related regulations.
6. To ensure and maintain a safe work place as well as promote safe work culture.
7. To maintain a working environment that is safe and without health risk with adequate
welfare facilities and resources.
8. To carry out investigation and trend of accident, near miss, dangerous occurrence,
occupational poisoning or disease that occurs at the place of work and submit a report
with recommendation for preventive and corrective action.
9. Inspect the place of work at least once in every 3 months.
10. Review the effectiveness of safety and health program and promote a safe work
conduct at the place of work.
