JOINT AFFIDAVIT OF DISCREPANCY

WE, REMY B. BERMEJO and ROGER B. BAYHON both of legal age,

Filipinos and residents of Barangay Agbalo, Panay, Capiz after having been sworn to an
oath in accordance with law, do hereby depose and say:

1. We personally know LADY MAE ARTAS BACEA, of legal age, Filipino,

sinbgle and a resident of Barangay Agbalo, Panay, Capiz;

2. She was born to parents Melody Artas Bacea and Lody B. Bacea on December
24, 1997;

3. Lady Mae Artas Bacea and her parents is our close family acquaintance;

4. This affidavit is executed to attest to the truthfulness of LADY MAE ARTAS

BACEA’S true identity as well as the true and correct spelling of her middle
name;

5. We know for a fact that the person by the name of “LADY MAE ARTAS
BACEA” appearing in the Certificate of Live Birth with Registry No. 2005-958
issued by the Philippine Statistics Authority and the name “LADY MAE ARJAS
BACEA” appearing in her BIR TIN/ Registration refers to one and the same
person;

6. We further attest that the foregoing discrepancy is due to a typographical error;

7. This affidavit is executed to attest to the truth of the foregoing facts, inform the
proper agency and for all legal intents and purposes.
