SUMMARY REPORT

SPACE SHUTTLE LAUNCH JAN 28,1986

NASA is now at work on its big project of space shuttle to be launched on January 28,1986.

Failure modes and effects analysis was mainly focused by NASA in order to ensure reliability. They have
conducted hazards analyses performed by shuttle engineers. NASA meteriologists sent weather
balloons to check cross winds and there was nothing unusual balloons were not drifted.

The design of space shuttle was shaped by engineering considerations likely the orbiter, external fuel
tank and solid fuel rocket boosters. Managers and Engineers conducted low temperature tests for the
use of O rings and made sure that launch is not to be taken in cool weather conditions. They made sure
that there is no faulty sealing system which would allow exhaust flames from solid fuel rock boosters to
vent directly on external tank rupturing the tank and causing explosion.

Finally the day has arrived where temperature conditions were tested which was perfect for the space
shuttle launch and at 11.30am challenger space shuttle consisting of five nasa austronauts and two
payload specialists was launched and the launch turned out to be successful.
