Palpation
13.Palpate the apical impulses and note its location.
14.Palpate the left parasternal borders for right ventricular
thrills.
15. Palpate the second left intercostal space for pulmonary
artery pulsation.
16.Palpate the suprasternal notch for aortic pulsation.
AUSCULTATION(with head of the bed up 30-45 degree)
17.Listen to the following area of pericardium with both the
bell and diaphragm:
Second right intercostal space
Second left intercostal space
Erb’s point
Tricuspid area
Apex
18.Listen for:
S1 and S2
19.S3 and S4 gallops
20.Listen for other sounds during systole and diastole:
murmurs,rub.
21.Ask the patient to roll onto left side. Identify the apex by
palpation.Using the bell listen the apex.This will bring out
mitral valve abnormalities.
22.Ask the patient to sit up and to inhale,exhale deeply,and
then lean forward. Listen over the aortic and pulmonic areas
for the diastolic murmur of aortic insufficiency and pericardial
rub.
Peripheral vascular exam
23. Inspect lower extremities for size, symmetry, color.
24.Palpate lower extremities for edema.
25.Palpate lower extremities for arterial pulses.
