ADH Upgrades to DCIS Excision
ALH/LCIS Uncommon to upgrade Close follow up

Excision if:
1) radiology-CNB pathology concordance
2) Other high risk lesions present (ADH,
papilloma, radial scar)
3) Pleomorphic LCIS
4) LCIS with necrosis
5) Nonclassic variants
DCIS High risk BCT + RT:
1) Negative margins (≥ 2mm adequate)
2) Localized to one quadrant
3) <4 cm
4) Grade (not comedo)
5) No contraindication to radiation
- OR mastectomy + SLNB
- Tamoxifen/anastrozole (ER+)
- Tamoxifen in ER- by patient choice to
prevent future ER+ lesions
