JUDICIAL AFFIDAVIT

BRICHEL MAE LASAM

This Judicial Affidavit of Brichel Mae Lasam, the Finance Manager, is

executed to serve as her direct testimony in the instant case.

This Judicial Affidavit is being offered to prove:

A) All the allegations in the Petition including all annexes appended thereto
and which were already marked as exhibits during the Pre-Trial of this case;
B) All other related matters, facts and circumstances relevant and material to
this case.

This Judicial Affidavit was taken at the office of Atty. Warlito Ebarle Labis
at 4A Macrohon Building, Campaner Street, Antipolo City, Rizal.

Questions were propounded by Atty. Warlito E. labis and these questions are
numbered consecutively, and each question is followed by the answer of the
witness.
1. Do you swear to tell the truth and nothing but the truth?

Answer: I do.

2. Are you aware that you may face criminal liability for false testimony or
perjury if you will not tell the truth?

Answer: I am.

3. Please state your name, age address and occupation?

I am Brichel Mae Lasam, 25 years old, single, and residing at #508 Kalaw
St. Brgy. Pinagsama Taguig City.

4. Are you the same Brichel Mae Lasam, the Finance Manager in this case?

Answer: Yes.

5. Do you know the Respondent in this case, Bella, Cathy, & Donna Partnership?

Answer: Yes, they are our client.

6. What type or form of business they are engaged in?

Answer: They are engaged in manufacturing, selling, and exporting handicraft

products.

7. When was the first transaction?

Answer: It was on January 9, 2019

8. Did they pay the purchase price? How much is the total amount?

Answer: No. Actually, the purchased price amounted to P500, 000.00.

9. It shows that based on the affidavit they made second, third and fourth
transaction, can you state what is the total amount of each transaction?
Answer: The second transaction amounted to P300, 000.00. Third transaction
amounted to P563, 104.22 and the last transaction amounted to P200, 000.00. The
total amount due is P1, 563,104.22. ANNEX C

10. Did you send them the statement of their account?

Answer: Yes, I sent them the Statement of Account on different dates to bring
notice that the payment of the handicraft products stands due.
11. After sending the Statement of Account, were they able to acknowledge the
letter?

Answer: Yes, but unfortunately, the notice which had sent to them remained
unheeded.
