he/she executed the same while of sound and disposing mind and memory;
that he/she has published unto us the foregoing will consisting of ________
pages numbered correlatively in letters in the upper part of each page, as
his/her Last Will and Testament and has signed the same in our presence at

thereof, and we, in turn, have witnessed and signed the same in each and
