I, YOLLY T. LEBANON, of legal age, Filipino, married

with postal address at General Santos City, after having been
duly sworn to in accordance with law, do hereby depose and
say:

1. That I am the attorney-in-fact of San Lorenzo Ruiz

Fishing Industry, Inc., the buyer of three (3) parcels
of land situated in Brgy. Tambler, General Santos City
registered in the name of Joel C. Congson;

2. That I have in my possession original copy of Extra-

Judicial Settlement of Estate Among Heirs of Joel C.
Congson with BIR annotation at the back;

3. That said copy of Extra-Judicial Settlement was among

my other documents placed in an envelope at the
office;

4. That said document was lost and could no longer be

found despite diligent efforts to locate the same;

5. That I am executing this affidavit to attest to the

truth of the foregoing facts and for whatever
purpose this may legally serve.

General Santos City, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am a holder of Plane Ticket route: Manila-Kuala

Lumpur-Manila, which I purchased through Marazul
Travel, Inc.-Marisa Bautista;
2. That the just recently, when I looked over for said
plane ticket, the same could not be found;
3. That diligent efforts were exerted to locate my said
plane ticket, proved futile as the same could no longer
be found or located;
4. That I am executing this affidavit to attest to the
truth of the foregoing facts and for whatever purpose
this may legally serve.

bank book, proved futile as the same could no longer be
found or located;
4. That I am executing this affidavit to attest to the
truth of the foregoing facts and for whatever purpose
this may legally serve.
