REQUIREMENTS FOR ORIGINAL APPLICATION FOR AUTHORITY TO OPERATE AS


CARGO SALES AGENT/ BREAKBULK AGENT

1. Verified Application in prescribed CAB form (with Cashier’s stamp of payment)

NOTE:
a. Filing fee: Php14,544.00
b. Notice of Hearing (signed by the ACASFAD Chief) shall be issued to applicant
and should be published at least once a week for two (2) consecutive weeks in a
newspaper of general circulation.

2. Affidavit of Publication (proof that the Notice of Hearing was published)


3. If Corporation: Certificate of Registration/Incorporation from SEC & Articles of
Incorporation; If Sole Proprietorship or Partnership, Registration of Business Name with
the Department of Trade & Industry
4. If Corporation, Latest General Information Sheet; If Sole Proprietorship or Partnership,
bank certificate(s)

NOTE: Paid-up Capital should be at least Php1M; If Sole Proprietorship or Partnership,


bank certificates showing at least Php1M;

5. Insurance Policy covering Office/Property (i.e. Fire, Comprehensive General Liability,


Property and All-Risk, etc.) and the corresponding proof of premium payment made
6. Original copies of Surety Bond Policy and the corresponding Official Receipt of
premium paid (issued in favor of CAB in the amount of P 200,000.00)
7. Breakbulk Agency Agreement with Foreign Agent or Cargo Agency Agreement with
Local Licensed Airfreight Forwarder (CAB Certified)
8. Audited Financial Statements or Audited Pre-Operating Financial Statements

 Only applications with complete and valid documentary requirements shall be received
by the Board. The application shall be given due course by this Board within three (3)
working days from the completion of all the requirements, including the requirements
of publication and ocular validation.

Note: All documents are required to be certified as authentic by applicant and are subject to comparison with
the originals. 1% legal Research Fee included, as mandated by Pres. Dec. No. 1856, dated 26 December 1982
