BAG TECHNIQUE AND DRESSING CARE WOUND

1) Prepares plan of home visit following the prescribed format.

2) Identifies the client properly. (to make sure that the person’s who will receiving care is the
right client)

3) Introduces self and explains purpose of visit and procedure. (to avoid the client from being
anxious)

4) Place the bag on the table or any flat surface lined paper/plastic lining, clean side out
(folded part touching the table). Put bag’s handles/strap under the bag. (to avoid from
being contaminated)

5) Ask for a basin of water and a glass of water if faucet is not available. Place these outside
the work area. (to lessen or prevent the microorganisms from spreading)

6) Open the bag, take the paper/plastic lining and spread over work field or area (the clean
side out). The paper lining, clean side out (folded part out). (to put your tool bag in a clean
surface with a plastic/paper make sure that these cover will prevent the bag from being
contaminated)

7) Take out hand towel, soap dish and apron and place them at one corner of the work area
(within the confines of the linen/plastic lining.) (to prepare hand washing, avoid from
commination and conserve time)

8) Do hand washing. Wipe dry with towel. Leave the plastic wrappers of the towel and soap
dish in the bag. (To avoid spreading of microorganisms after were used)

9) Put on apron right side out and wrong side with crease touching the body, sliding the head
into the neck strap. Neatly tie the straps at the back. (to protect the nurse’s uniform)

10)Put out things most needed for the specific case and place at one corner of the work area.
(to have them readily accessible)

11)Place waste paper bag outside of the work area. (to avoid other unused equipment from
being contaminated and prevent from spreading of microorganisms to the client)

12)Close the bag. (To avoid contamination)

13)Proceed to the specific nursing care or treatment. (to give comfort and security and hasten
recovery)
14)Assemble all equipments and bring to bedside. (to have them readily accessible and for
the smooth flow of procedure)

15)Explain the procedure to the client. (To prevent the client from being anxious)

16)Do the medical hand washing. (to prevent cross contamination)

17)Position the client according to site of the wound. (to make them comfortable and lessen
the pain during treatment)

18)Expose the wound and drape the area. (to easily clean the wound area)

19)Undo materials (binder or adhesive) and secure the dressings. Lift dressing off by touching
the outside portion only. If it is soiled, use forceps. (to avoid contamination and infection)

20)Clean the wound with antiseptic solution prescribed as necessary.

A. Aseptic wound – start from incision (consider the cleanest part) then away from
it. (to prevent spreading the microorganism to wound and avoid from infection)

21)Apply the ointment thinly as prescribed, if any on the wound and on the skin if drainage is
present.

22)Cover the wound with sterile dressings and secure in place with adhesive applied at
edges.

23)Arrange client’s clothing and keep him/her comfortable.

24)After completing the nursing care or treatment, clean and alcoholize the things used. (to
prevent contamination and infection for the next day visits and before were use the things
to client)

25)Do hand washing again. (To protect the caregiver and prevent infection)

26)Open the bag and put back all articles in their proper places. (to avoid from being
contaminated)

27)Remove the apron folding away from the body, with soiled side folded inwards and the
clean side out. Place it in the bag. (To avoid nurse’s uniform from contamination)

28)Fold the linen/plastic lining; clean; place it in the bag and close the bag.
29)Documentation: Record and chart time, dressing appearance of wound with amount and
character of drainage (if any), observation, treatment such as removal or insertion of
drains, packing, etc.

30)Make post-visit conference on matters relevant to health care, taking anecdotal notes
preparatory to final reporting. (for reference in the next visits)

31)Make appointment for the next visit, taking note of the date, time and purpose. (for the
follow up care)
