Request letter..

I am writing this letter to make a formal request to have access with the LIS of EOSY 2019-2020. This is
to update the status of the Student Cruzado Jessie Cabungcal and Catapang Robinson Gomez of IA 11-
Poseidon. Which were tagged as Incomplete. The said students were given the chance to complete their
requirements to pass the subjects, however due to certain circumstances such as the Taal and Covid-19,
the student’s School forms, had not been updated properly.

In addition, we would also like to request for Samonte Crisanto Delas Alas, of IA 11- Poseidon, which was
mis tagged as a Completer instead of Dropped, which was an honest mistake by the adviser Angelo
Villafranca as the name was confused with another student. The student’s last attendance was July 16
and has dropped due to lack of interest.
