Description

While working in the private ward. I came across an incident which being forced

me to think about the responsibilities, accountability and care provided by nurse. During

my rotation, I was assigned to care for the ventilator patient. Patient had GCS 10/10

tracheotomy is in place and on SIMV mode. Patient is able to communicate through

writing. On assessment it has been found that, Patient was unkempt, hair lice were there,

skin is dry and scaly, untrimed nail ,suction tube was uncover and urine bag was lying on

the floor. Staff was there but ignoring the patient needs.

Feelings

On the above situation, I felt very upset and surprised to see that the patient was

going towards hospital acquired infection, just because of negligence and lack of

knowledge of staff. I also felt that this negligence and lack of knowledge can lead to

increase hospital stay, making patient more prone for infection. We are as a nurse talking

about the holistic care and are unable to give basic physiological care.

Evaluation

This incident stood out because of negligence of staff, lack of knowledge regarding

Asepsis technique, infection control and knowledge about the basic need of the patient

among nursing staff. This can lead to unsafe practice and low quality patient care that can

endanger patient life’s and make prone to get hospital acquired infection. In addition,

their supervisors were also not critical observer to correct these harmful practices. I asked

my self what could be my role? How I can bring change in the care giver attitude? How

can I help the patient?

 Analysis

After analyzing the situation, we can make the assumption that the cause of negligence

behavior might be because the staff had many others work to do, their seniors are doing

the same role modeling as they are doing. They are the unaware of the concepts of

infection control and care of dependent patient. Further more the staff could have assess

the patient individual needs and available resources to provide optimum care for the

patient. On other hand the assign nurse could have done her assessment and provide the

care as needed by patient to minimize the infection and complication. Hospital authorities

could develop their infection control plan and could develop standard nursing care which

can improve quality care of patient .The supervisors could reinforce their nursing staff,

educate them and can become role model. I as a nurse have tried to be role model in

patients care. I took nursing student and assign staff and involve them in care of patient .I

thought them how they prevent the patient from infection.

Conclusion

This incident helped me learn, what can be essential steps in managing the patient

to minimize harm to patient's life and enhance safety. Further more I learnt that how can

small preventive tips of a nurse can help patient be safe from infection. I recommend that
