I, ARCADIO DACLES, ___ years old, Filipino, married, male, and a resident of Brgy.
Jebaca, Maayon, Capiz after having been duly sworn to in accordance with law do hereby depose
and say:

PRELIMINARY STATEMENT

The person examining me is ATTY. MARY ZEN DIVA with office address at Borres
Law Office and Associates, Fortune St. Banica. Roxas City. The examination is being held at the
office of the said lawyer on ___________ at 10 o’clock in the morning. The questions are asked
in the English Language but are translated in the Hiligaynon dialect which I speak and fully
understand.

I am answering the questions asked of me, fully conscious that I am under oath, and that I
may face criminal liability for false testimony or perjury.
