AFFIDAVIT OF SERVICE

I, BONIFACIO P. VICENTE III, an employee of the Zamboanga City Water

District with office address at Pilar Street, Zamboanga City, Philippines, under oath,
hereby depose and states:

That I am a regular employee of Zamboanga City Water District, located at Pilar

St., Zamboanga City, with the position Plumber B;

That on August 12, 2014, my supervisor had instructed me to deliver a FINAL

DEMAND with ORDER OF CLOSURE to ZCWD customer JOE, LOLLY with
Account No. 0254325, located at Tomas Claudio St., Zamboanga City;

That at about 10:00 a.m. of August 12, 2014, I proceeded to the address
abovementioned in order to make the delivery, however, when I handed the document to
the person present at the place of delivery, the person refused to receive;

That I just marked the demand letter with the wordings “REFUSE TO
RECEIVE” and handed the document back to my supervisor, informing him that it was
refused by the person present at the place of delivery.

That I am executing this affidavit in order to attest the veracity of the foregoing
facts and for whatever legal purpose that it may serve.
