REFLECTION PAPER 4B

Emergency cesarean section is done when there is fetal or maternal distress, prolapsed umbilical
cord, maternal hemorrhage, placental abruption, or uterine rupture. A Pfannestiel incision was performed
in the video. However, there was no demonstration of the incision shown in the video. The reason for
using this incision was also not explained in the video. The video started in the middle of incising the
subcutaneous layer. There was no demonstration of the sterilization of the surgical site using iodine.
Proper draping of the uninvolved sites of the patient was also not showed in the video. The camera was
unable to capture some of the steps in the procedure. The camera angle was either not focused on the
surgical site or covered by one of the operators. The peritoneum was opened after cutting the
subcutaneous layer. Sterile gauze pads were inserted underneath the uterus inside the peritoneum. There
was no explanation for the purpose of this step. The uterus was opened but the video was unable to
properly show the incision done. A low segment incision was performed to open the uterus. Once the
incision is wide enough, the delivery of the fetal head is initiated. The operator carefully inserted her hand
inside the uterus to lift out the fetal head. Afterwards, the shoulders were lifted out and the baby was
delivered. Drying of the baby and cord clamping was performed. Traction was applied on the umbilical
cord to assist in the delivery of the placenta. The operator pulled on the umbilical cord and the placenta
was delivered. The bleeding was wiped off using a sterile gauze pad. The uterus was clamped but the
purpose of that step was not explained in the video. The uterus was repaired but the video was unable to
properly demonstrate the suturing. The camera was not in focus of the surgical site most of the time. At
the end of the video, the words of doing the abdominal muscle repair and skin repair were flashed on the
video. There was no demonstration of the actual repair of the abdominal muscles and skin. The video
ended abruptly without showing the entire procedure. The video started after the start of the procedure
and stopped before the end of the procedure. Most of the procedures performed were unseen because
the camera was out of view. I learned that it is important to be quick to react in a procedure like this.
During the repair of the uterus, there was a lot of bleeding. It is important for us to be ready and react
correspondingly when situations such as that occurs during the operation. I learned that it is important to
position one’s self properly when performing a surgical procedure. When performing a procedure, one
must know every step from the beginning to the end.
