PARENTAL CONSENT FORM

ALIKMATA Productions is a production team from the class of 2 COM 5 from the
University of Santo Tomas. In line with this, we have begun production on our first film,
a documentary film which is yet to be titled. The said documentary will be taking on the
economic and cultural issues brought about by the misconceptions from the African
Swine Flu epidemic, as well as the families of those affected by this issue. This
documentary film is also in partial fulfillment in our CA3283 class, Broadcasting
Principles.
The team has begun production for this documentary film and certain dates have
already been scheduled for shooting. With this, we are requesting the attendance of
your son/daughter, __________________ and his/her utmost cooperation on this
production. Below are the details of our shoot:

Where: Sta. Maria, Bulacan
When: November 28, 2019 – November 30, 2019

Rest assured that we will be in charge of your son/daughter’s welfare and will be
responsible for his/her needs (i.e. Food, Accommodation, and Transportation)
throughout the duration of the production. If you could kindly answer the slip to inform
