    May I request for deletion of contribution under SSS ID Number 33-


    127561-9 for the late CRISALDO GELIO SALAZAR who passed away last
    February 25, 2020 and was reported SEPARATED from the company of same
    date of his demise please see attached Certificate of Death for reference.

    A slight misunderstanding with our previous Admin Staff (no longer


    connected with us) prior to pandemic arise thus failing to revise nor correct said
    contribution made for the month of MARCH, APRIL and MAY 2020
    respectively in favor of High Profile Security Agency Incorporated under
    employer ID number 03-9160581-3 hence our appeal for the deletion of
    contribution made intended for the late Crisaldo Gelio Salazar for ease
    transaction of death claim etc.

    Hoping that this request would be granted and would really appreciate
    much if action would be taken the soonest possible.
