Attached to cardiac monitor and pulse oximeter, O2
sat 100% attached to O2 @ 2L/m via nasal cannula.
Vital signs closely monitored.
Prepared for induction of anesthesia.

8:55AM Placed in L lateral position.


Epidural anesthesia induced by Dr. Lagumbay
Desired aeffect of anesthesia effect noted.
Initial checking and counting of instruments done.
Draped aseptically.

9:00AM Procedure started by Dr. Soyaso assisted by Dr.


Mayumi.
Bleeders clamped, fractures exposed and reduced.
Edges curetted.
Peroxide wash done.

10:15AM Intra-op X-ray done, film seen by Dr. Lagumbay.


Screws applied, JP drain applied.
Final count of sponges and instruments done.
Relayed to the surgeon. Skin closed with staple wires
Dressing applied.

10:45AM Procedure ended. Transferred to recovery room.


Kept cleaned and dry.
Chills noted. Blanket provided.

11:00AM Blood transfusion w/ blood type O.


Started and regulated.
Bought to the PACU, awake, with IVF of PLR 1L
@850cc, infusing well.
BT ongoing, with FC in place.
Latest VS: T=37.6, P=89 bpm, RR= 22 Bresths
per/min, BP=120/80mmhg.

12:00NN Endorsed to NOD.
