1. Patient’s Profile.
Mr. M is a 41 years old, male, Single, a Lawyer and a Roman Catholic, He ruptured his TA
(the first time). Treatment consisted of full leg plaster followed by outpatient physiotherapy.
A while ago he was walking on level ground when it re-ruptured. Previous diagnosis had
been Achilles Tendinopathy.
2. Psychological
Information about pt. family wasn’t given. According to Erik Erikson's Stages of
Psychosocial Development he is under the Generativity vs. Stagnation stage, He is a lawyer
working in city and travels in by underground. He is single and lives alone in a first floor
condominium.
3. Mental and Emotional Status
The patient is conscious, he revealed what an extremely difficult time he was having
adapting to this long period of inactivity.

4. Environmental Status
Mr. M aware of his environment, conversant, oriented to self and others around. He is aware
of the time, date, and place.
5. Sensory Status
There is no known visual deficit like color blindness. Mr. M height wasn’t indicated, no eye
defect stated and clearly hears and understand words spoken.
6. Motor Status.
Pt. has limited ROM and uses crutches.
7. Nutritional Status
Not given.
