Cranial nerves examination

Inspection
⁃ scar: craniotomy, tracheostomy neurofibroma
⁃ Facial asymmetry
⁃ Ptosis, proptosis, eye deviation
⁃ Consciousness
Olfactory
⁃ ask whether got problem with smelling
⁃ Dont use pungent smell objects like lemon etc
Optic
⁃ Pupillary light reflex (direct, consensual)
⁃ Relative afferent pupillary defect (RAPD)
⁃ Visual acuity - Snellen chart
⁃ Colour
⁃ Visual field - finger move inwards like snowflake shape
Oculomotor, trochlear, abducens*
⁃ H test
Trigeminal
⁃ Corneal reflex (cotton wool)
⁃ Face both sides (superior, midface, inferior): cotton & pin prick (try on
sternum first)
⁃ Feel masseter while clenching teeth (ketap gigi)
⁃ Open mouth and dont let me close it (push chin upwards) - pterygoid
⁃ Jaw jerk
Facial* (bell’s palsy)
⁃ Ask sensory (tongue ant. 2/3 & ear)
⁃ Asymmetry
⁃ Lift eyebrow
⁃ Close eye dont let me open it
⁃ Smile (show teeth)
⁃ Blow (muncungkan mulut)

Vestibulocochlear
⁃ whisper
⁃ Weber (forehead)
⁃ Rinne (mastoid and air)
Conductive deafness: bone louder than air
Neural deafness: both reduced

Glossopharyngeal, vagus
⁃ Say “aaaa” until can see uvula
⁃ Look for deviated uvula (deviate towards normal side)
⁃ Cough, hoarse voice
⁃ Swallow
⁃ Gag reflex
Accessory
⁃ lift up shoulder and resist
⁃ Turn head to the side, resist when push chin
Hypoglossal
⁃ tongue muscle wasting, fasciculation, deviation (to affected side)
⁃ Press tongue against face
