I. Identify / analyze two medication related problems

Problems:
 Unintentional administration of insulin instead of influenza vaccine at a public-
school clinic in Missouri.
 The patients who were injected by insulin complained and the two nurses are aware
of what was happening but continued to administer the “vaccine” which was the
influenza.
 How the medicine was stored.

Alternative solutions:

 Discontinue administering the vaccines since two patients already complained.


 Appropriate storage of the drugs and rechecking of the drugs before administering.
