F- Acute Pain

D- my son had an electric shock and has burns on his fingers” as verbalized by father, cooperative, and in
no acute distress, burns noted on the fingertips of the right second and third fingers, another on the
dorsum of the left hand, and a third on the chest, about 22% burns noted, vitals are stable.

A-- Assessed the level of pain frequently using pain scales, administered analgesics before all dressing
changes and burn care, elevated burned extremities periodically, covered burns as much as possible,
changed child’s position frequently, provided diversional activities, provided basic comfort
measures: massaged of uninjured areas, frequent position changes, encouraged verbalization about pain
educated parents on benefit of rest and uninterrupted sleep of their child, encouraged use of stress
management

R- patient verbalize adequate relieve of pain.

FDAR 2

F- Impaired physical mobility

A – assessed circulation, motion, and sensation of digits frequently maintained proper body alignment
with splint especially for burns over joint, performed ROM exercises consistently, medicated for pain
before activity or exercise, encouraged for parent support and assistance in performing activities of daily
living, encouraged patient participation all activities as individually as possible.

R- patient maintained maximum range of motion without contractures.
