    I hereby declare and confirm that I am a Non-Resident Indian and I am presently on contract
    with_________(Company registered)____ in____________________ (address of the
    principal)____________________

    I also confirm that I will inform the Bank, in case:


     I do not renew my contract or choose to go on a new contract OR
     I am unable to proceed on a new contract or in any case in the event that my status of Non-
    Resident Indian is altered.
    Accordingly, I will have the Non-Resident accounts opened in my name re-designated to Resident /
    RFC accounts (as applicable).
