    1. Account opening form dully filled and signed with stamp

    2. Account opening request (on company letter head with clear operating instructions) duly
    signed by authorized signatories with stamp.

    3. Updated list of directors (on company letter head) duly signed by authorized signatories
    with stamp

    4. Board Resolution with clear operating instructions duly signed by authorized signatories
    with stamp. (Format attached)

    5. Valid & attested CNIC or passport copies of all director and authorized signatories duly
    signed & stamped by Company secretary

    6. Signature cards duly signed by signatories with company stamp

    7. Latest Form 29 containing names of all directors. SECP certified copy duly signed &
    stamped by Company secretary

    8. Latest Form-A SECP certified copy. Duly signed & stamped by Company secretary

    9. Standard Board Resolution (on company letter head) duly signed by all signatories,
    company secretary with stamp.

    10. Certificate of Incorporation. SECP certified copy duly signed & stamped by Company
    secretary

    11. Memorandum & Articles of Association duly attested by SECP. Signed & stamped by
    Company secretary

    12. W8BEN-E signed and stamped by authorized signatories (last Page)

    13. Sign differ Form incase signature differ from CNIC

    14. NICOP undertaking required from NICOP holders only.
