Consumer’s Use of Informatics

1. Define and discuss mobile health.

The World Health Organization defines mHealth as “medical and public health practice
supported by mobile devices.” Mobile devices include smartphones and tablets, as well as
devices that provide real-time patient monitoring like FitBits and other wearables. As new
applications of mHealth are discovered and existing applications are improved, patients are
being given even more insight into and power over their own health and healthcare.

Mobile health technology is all around us and here’s a few scenarios:

 A fifteen-year-old patient with diabetes uses an app on her smartphone to enter her
glucose readings, earning points for readings within the desired range. She uses the
same app to play games that reinforce ways she can help manage her diabetes with her
diet.

 A new dad emails a picture of a worrisome rash to his daughter’s pediatrician to

determine if she needs to be seen in the office or if the rash can be treated with an over-
the-counter medication. The dad is then able to speak with his doctor from the comfort
of his own home using telemedicine.

2. Explain the advantages and its applications. Why mobile health is good?

 Gives patient faster access to providers and care. Unlike traditional and inefficient
telephone access to physicians and healthcare organizations, mobile health technologies
enable patients to quickly send secure messages, schedule appointments, and connect to

providers 24/7 for telemedicine visits. In fact, telemedicine is one of the fastest growing
ways patients are using mHealth apps on their mobile devices.

 Improves medication adherence. Once patients are discharged from the hospital or
leave the clinic, it’s tough to control whether or how they take their medication,
mHealth apps and mobile technology solutions improve this by giving patients features
such as automated medication and refill reminders, and educational information that
improve the chance they’ll take their medications properly.

 Makes remote patient monitoring possible and easy. Remote patient monitoring is a
subcategory of home care and telehealth that enables patients to use mobile devices and
mobile health technology to gather, enter, or automatically collect health data and
transmit it to healthcare providers. The ability to track patient conditions in between
visits, and notify a clinician or care manager when the patient’s biological data is
putting the patient at risk, is a big step forward in managing care.

 Increases medication reconciliation accuracy, which improves patient safety. The

fact that many patients cannot accurately recall their medication list to a physician, ED
intake or hospital admission nurse, or other provider, puts the patient at significant risk.
The Cureatr Meds 360 solution gives providers and other clinicians a bird’s-eye view of
all medications a patient has been prescribed - as well as pick up and refill history,
dosage changes, and more - on an uncluttered, iPhone or Android screen display.

 Improves provider communication and coordination. Given most providers’ full

appointment schedules, long days, and multiple facility coverage requirements,
connecting with colleagues, coworkers, and patients is challenging. Mobile health

technology improves the ability to connect across the health system and with referring
physicians and office staff using secure messaging and texting, mobile health record

3. Identify available international organization resources. Where is mHealth used?

Mobile health, or mHealth, uses mobile technologies as tools and platforms for health research
and healthcare delivery. Although cellphones and other new technologies are increasingly used
in research and health care, very limited data are available to determine their impact. Fogarty's
Mobile Health: Technology and Outcomes in Low- and Middle-Income Countries program
aims to support multidisciplinary teams to research possible new mHealth tools or interventions
either for chronic diseases or for an array of other health issues, emphasizing the evaluation of
health-related outcomes. The collection of other online resources below includes publications,
mHealth and bioinformatics software applications, and nonprofits, educational institutions and
companies supporting mHealth efforts.

mHealth (mobile health) is a general term for the use of mobile phones and other wireless
technology in medical care. The most common application of mHealth is the use of mobile
devices to educate consumers about preventive healthcare services. However, mHealth is also
used for disease surveillance, treatment support, epidemic outbreak tracking and chronic
disease management.

Ambulatory Care and Administrative Applications

1. Describe the state of art in using information systems in ambulatory care.

The central purpose of an ambulatory care information system is to communicate information

to the practitioner to facilitate clinical decision making. The clinical decision can be considered
the dependent output variable in a process having the information system, the patient, clinician
characteristics, and the environment as the independent input variables. Evaluation approaches
using patient outcomes are problematic because of the indirect relationship between the
information system and patient outcomes. Some process measure techniques lack validity
because they do not represent a generic sampling of clinical decisions made in ambulatory care.

2. Describe various roles for nurses in ambulatory care.

 Registered Nurses enhance patient safety and the quality and effectiveness of care
delivery and are thus essential and irreplaceable in the provision of patient care services
in the ambulatory setting.

 Registered Nurses are responsible for the design, administration, and evaluation of
professional nursing services within an organization in accordance with the framework
established by state nurse practice acts, nursing scope of practice, and organizational
standards of care.

 Registered Nurses provide the leadership necessary for collaboration and coordination
of services, which includes defining the appropriate skill mix and delegation of tasks
among licensed and unlicensed health care workers.

 Registered Nurses are fully accountable in all ambulatory care settings for all nursing
services and associated patient outcomes provided under their direction.

3. Provide an overview of implication and applications of information technology.

Applications of Information Technology

Every day, people use computers in new ways. Computers and other electronic devices are
becoming increasingly affordable. They continue to be more powerful as information-
processing tools as well as easier to use. Humans are continually becoming dependant on IT-
enabled devices for carrying out simple tasks like remembering a phone number to complex
ones like flying a fighter plane. Information Technology has applications in almost all aspects
of our life. Some of the important ones are:

 Science and Engineering: Scientific progress in fields like biotechnology is almost

entirely dependent on the use of computers and other microprocessor-controlled
devices. Using supercomputers, meteorologists predict future weather by using a
combination of observations of weather conditions from many sources, a mathematical

representation of the behavior of the atmosphere, and geographic data. Computer-aided

design (CAD) and computer-aided manufacturing (CAM) programs have led to
improved products in many fields, especially where designs tend to be very detailed.
Computer programs make it possible for engineers to analyze designs of complex
structures such as power plants and space stations.

 Business & Commerce: One of the first and largest applications of computers is
keeping and managing business and financial records. Most large companies keep the
employment records of all their workers in large databases that are managed by
computer programs. Similar programs and databases are used in business functions like
billing customers; tracking payments received and payments to be made; and tracking
supplies needed and items produced, stored, shipped, and sold.

 Education: The advent of Information Technology has changed the meaning of the
term “literate”, with computer literacy being almost as important as basic literacy in
many cases. Computer education is an essential course at the primary level in most
schools across the world. With more information getting digitized every day, and the
internet making it accessible to anyone across the world, students are increasingly
relying on electronic sources of information rather than physical libraries for their
needs.

 Governance: The concept of e-governance is one of the most novel applications of

Information Technology whereby it is changing the lives of millions across the globe.
Computerization of Government activities makes it easier to supervise and audit, and
makes the administration more responsive to the needs of society. It also bridges the
divide between the Government and the people.

 Medicine: Information Technology plays an important role in medicine. For example, a

scanner takes a series of pictures of the body by means of computerized axial
tomography (CAT) or magnetic resonance imaging (MRI). A computer then combines
the pictures to produce detailed three-dimensional images of the body's organs. In
addition, the MRI produces images that show changes in body chemistry and blood
flow. Most critical life support equipment are programmed to respond to changes in the
patient’s status in split-seconds, thereby reducing the response time and risk of human
error.

 Entertainment: IT has changed the lifestyle of most people. The convergence of

various technologies has created various options for entertainment like games,
streaming music and video, digital television broadcasts, satellite radio, animated
movies etc. which can be accessed with the help of mobile phones, PDAs, notebook
computers or on television either with a cable connection or wirelessly using newer-
generation WiFi, CDMA or GPRS technologies.

4. Discuss the need for nursing data standards.

In the context of health care, the term data standards encompasses methods, protocols,
terminologies, and specifications for the collection, exchange, storage, and retrieval of
information associated with health care applications, including medical records, medications,
radiological images, payment and reimbursement, medical devices and monitoring systems, and
administrative processes.

5. Describe the benefits of administrative IT for nursing.

Recent technological advancements have drastically changed how care providers manage data
and deliver services. For this reason, both entry-level and experienced nurses continually learn
new technologies, while nursing schools develop innovative curriculums that serve the new
caregiving environment. The most notable changes have occurred in the field of healthcare
information technology (HIT), where providers use powerful software and hardware to manage
patient records. By combining HIT with electronic health records (EHRs) and administrative
data, organizations perform more efficiently. Practitioners of informatics use HIT to maximize
the utility of EHRs to store, analyze and make critical decisions using real-time patient data.
The following benefits represent the latest operational improvements realized through health
information technology.

 Increased Patient Safety

Care providers that have implemented HIT and EHR systems have experienced
significant safety improvements due to the real-time information accessibility afforded
to all stakeholders. Additionally, these technologies allow researchers to analyze
information with extraordinary detail and accuracy.

 Improved Scheduling

Many unforeseen and counterproductive circumstances may arise during a typical care
provider workday, which is compounded when staffing shortages occur. To mitigate
this risk, healthcare managers use a technology called resource-demand management
that analyzes caseload information and staff availability. A similar technology,
workforce-management solutions, performs largely the same tasks, while also helping
managers control costs and service quality.

 Reduced Test and Procedure Redundancy

Electronic health records (EHRs) simplify patient information sharing; eliminating

redundant testing and procedures and reducing discomfort, inconvenience, medical
expenses and risks. Using this technology, patients can immediately authorize access to
timely and relevant information that is needed by their caregivers.

 Efficient Care Coordination

Easy access to information facilitates collaboration among multiple care providers. This
transparency helps to eliminate errors and risks, such as conflicting prescriptions and
treatments. Care providers use monitoring technology to trigger alerts when such
discrepancies arise in patients’ electronic health records.

 Faster Lab Results

Electronic health records allow caregivers to retrieve patient information faster than
conventional methods, facilitating timely test scheduling and treatment. Furthermore,
caregivers can access test results as soon as they become available.

 Increased Patient Information Accessibility

By law, American citizens have the right to access their own medical information to
check for, and correct, errors and omissions. To this end, some care providers offer
patients online access to their medical records. This practice increases information
transparency and in many cases enables patients to more quickly find answers to their
questions.

 Enhanced Performance Analysis

While EHRs greatly reduce drug errors, some mistakes still occur. Errors typically
happen while administering, dispensing, prescribing or transcribing medications.
Electronic information management allows caregivers to identify areas for improvement
and increase patient safety.

 Streamlined Records Administration

Electronic health records replace the paper charts historically used during care provider
visits. As more providers adopt EHR technology, patients will be asked less frequently
to fill out information during visits, because providers will already have current data on
file. Furthermore, EHRs streamline the traditional intake process, which can sometimes
seem excessive for relatively minor medical needs. EHRs also reduce errors by making
current patient information immediately accessible to all network-connected care
providers.

 Reduced Hospital Stays

According to the Office of the National Coordinator (ONC) for Health Information
Technology, electronic record implementation reduces the time needed to assess fall
victims by 20-percent, reducing patient stay times in those cases. Likewise, the National
Center for Policy Analysis reports that facilities using HIT systems reduce average
patient stays from 5.7 days to 5.5 days. This effectiveness has led researchers to explore
other benefits to using electronic health records and HIT, such as whether
telemonitoring can delay nursing home admittance among elderly patients.

 Reduced Operational Costs

 Cost reduction represents one criterion for satisfying the Triple Aim objective of the
Agency for Health Research and Quality. Healthcare information technology plays a
key role in significantly reducing operational costs. Caregiving organizations that
incorporate health information technology often see significant cost savings within the
first year of implementation.

6. Delineate the specific computer applications of nursing administration.

The use of industrial computer carts on wheels is common in healthcare. The modern industrial
computer cart is designed for the demands of hospitals and health centers. In particular, heavy-
duty medical carts have allowed for the widespread use of computers, which has improved
healthcare efficiency and patient care.

 Computers for Medication and Treatment

Computers have revolutionized the way medication is administered and diagnosing

medical treatment required. With access to accurate patient records, doctors and nurses
can review an individual’s full medical history in detail, enabling them to diagnose,
treat, and administer medications in a timely manner.

 Computers for Security and Organized Record Keeping

A computer network is a far more secure way to store and organize patient records.
Filing patient records in cabinets or on shelving is quickly being phased out, with
computer systems offering a more efficient solution. A computer keeps patient records

secure, organized, and easily accessible. A mobile computer cart gives doctors and
nurses access to records anywhere in the hospital.

 Computers for Every Room

Medical staff can quickly access patient records, or hook up other equipment to a
computer, to perform their duties and still have full mobility around a room.

 Computers for Offsite Care

Not all patients can access a hospital due to their health condition, which means
sometimes doctors and nurses have to go offsite to treat people. Laptop computers are
portable so can accompany doctors and nurses on home visits, allowing them to access
the hospital network even when not on site. Having access to patient information,
especially when not in the hospital is crucial for ensuring there are no errors when
treating a patient in their home.

 Computers for Surgical Procedures

The use of computers in operating theaters is helping save lives. Surgeons rely on
computer systems for performing intricate procedures and monitoring the wellbeing of
their patients. Mobile computer stands play a key role in the success of surgical
procedures.

 Computers for Communication and Inventory Management

Computers enable better communication across different hospital departments, allowing

them to quickly share updates or medical research. Doctors and nurses can also access

2. Nursing Informatics in Europe

In Europe, as in many countries worldwide, the main rational for implementing a greater use of
information technology (IT) in the healthcare sector is to improve safety and quality, improve
patient outcomes, and at the same time try to reduce costs of healthcare. •Europe is a continent
with over 750 million inhabitants in about 50 countries with many different languages, culture,
social system, and other living condition. This makes obvious that it is not possible to provide
the entire picture of nursing informatics in the area.

The main mission, in Europe, is to establish a stable infrastructure quality, facilitates the
reduction of errors, and the delivery of evidence-based and cost-effective care. Some core
building blocks of this are electronic health record (EHRs), nursing informatics education at all
level, communication and terminologies, and standards for technology, communication, and
patient care.

expected to communicate any information that can improvise and pace decision making by
patients, nurses and other consumers and health care providers. The process of nursing involves
four different steps. These are planning, execution, observation and assessment. However, since
information management is combined with the process of nursing and also with its practice, a
few nursing communities have added a new step in the process that is known as documentation.
When studied deeply, the documentation and specific patient-centered care have been identified
as the main role players in nursing processes. Automated documentation is very essential, and
not only for nursing, but for every patient care system. Updated and precise information about
the patient at every level of nursing can be the way to secure and better quality patient care.
There are a few things that need to be done in health care and nursing facilities for the
successful execution of the information system. Primarily, it is essential to have good,
specifically designed systems that can handle the nursing system in an organization. Secondly,
but of equal importance, is the acceptance and proper integration of the newly installed
information system into the normal workflow. Finally, the organization should also have
suitable resources that can maintain the above mentioned two factors.

Nurse informatics specialists are the best that a nursing facility or a hospital can get when it
comes to control nursing informatics. These nurses possess critical thinking skills as well as
high analytical skills. The nurses are also capable to understand the delivery workflow of
patient care and thus can find better integration points for the process of automated
documentation. Hence, it can be said that nursing informatics specialists are the best project
managers since they are the link between the nursing process and project management process.
To gain excellence in the field of nurse informatics, one should know about relational databases
and also be skilled in Microsoft Office, more importantly with Excel, Visio and Access.
