Mid-Term Evaluation

This mid-term evaluation is as a working tool for the Agency RT Supervisor and the student-
intern. The Agency RT Supervisor will complete the evaluation and then together with the
student provide an evaluation of the student’s clinical performance at the mid-point of the
internship experience. It is important that the Agency RT Supervisor’s evaluation is as accurate
as possible so the student clearly understands the areas they are meeting expectations and the
areas that are in need of continued improvement and they are not meeting expectations.

During the Mid-Term Evaluation of the student it is important that you indicate if the student is
on target or if there are concerns that need to be addressed with the agency supervisor. Please
complete the following evaluation thoroughly by rating each area indicated below and if
needed providing comments for each section. This will assist in determining the areas of
support that are needed for a successful completion of the internship experience moving
forward. This is a critical document for the student, agency, and university as it clearly outlines
the student’s abilities and performance.

Internship Completion
It is important that the student and the agency supervisor understand that successful
completion of an internship is twofold:
1. The student must receive a grade of a “C” or better in the required academic
coursework as outlined in the course syllabus.
2. The student must receive a “Successful Completion” on the Final Agency Evaluation of
Student-TRIE from the site supervisor.

The Final RT Agency Supervisor Evaluation of Student-TRIE asks the agency supervisor to
indicate one of the following:
 Successful Completion
 Incomplete
 Unsuccessful Completion

These are defined as follows:


Successful Completion: The student passes the internship experience and received grade
according to the points earned.

Incomplete: The student has not successfully met the agencies requirements for a successful
internship. The university supervisor will meet with the agency supervisor, to outline with the
student the specific tasks on the NCTRC Job Analysis that the student needs additional
experience and training to achieve “Successful Completion” for the internship. Together an
individualized plan for the student will be developed that includes specific outcomes and
additional hours required. The student will receive a “T” for a grade in the course, which
indicates the internship is still in progress.

Unsatisfactory: The student has not met the agency requirements for a successful internship. If
an agency supervisor records an “Unsatisfactory” on the student’s Final Agency Evaluation of
Student-TRIE the agency supervisor is stating the student is not able to meet the required
competencies of the agencies internship. The agency has met with the student throughout the
internship documenting concerns regarding the student’s abilities and performance, university
supervisor was notified of concerns, and student and university supervisor have discussed the
performance issues.

When given an unsatisfactory the agency supervisor is stating the internship experience is over
and the student has not met the stated guidelines of the agencies internship thus receiving a
“Unsatisfactory”. Additionally, the agency supervisor cannot verify that the student meets the
minimal competencies as outlined on the student evaluation, which references the NCTRC Job
Analysis. If a student receives an “Unsatisfactory” the student will not be able to receive higher
than C- grade and will need to repeat RECTH 5828 Recreational Therapy Internship. The
university supervisor and the student will meet to discuss the student’s options.

Indicating projected completion status on the Mid-Term Evaluation


At the end of this Mid-Term evaluation, the supervisor will be asked to indicate if the student is
on target for successful completion or if there are notable concerns. This will allow the
academic supervisor to provide the necessary support for the student and internship
supervisor.

Throughout her internship she has constantly been improving her knowledge of the RT process.
She stays late and studies for the exam and has scheduled a date to take her exam. She has
shown strong initiative to learn about the program during RT meetings.

Comments:
We have not had any admits to my caseload since Grace has been an intern. We will be
conducting assessments and review more medical charts in the upcoming weeks. Due to a recent
cyber-attack, we have been unable to use our online medical charts to review as needed. The
computers are back and we have begun to review clients history and charts.

Comments:
She has done well to help select interventions based on our Resiliency theory and Leisure Well-
being model. Due to us not completing an assessment for a resident yet, she has been unable to
select goals for the resident to work on while present at CHYC. She is aware of the population
and does well to ensure all materials are accounted for as well as assess the risks of materials
being used prior to running group

Comments:
She is able to tell what groups have stronger impacts than others. She has begun to take over
running groups and does well to keep the resident’s attention and ensures they remain safe in
group. She is vigilant at collecting and counting group materials as well.

Comments:
She has done very well with seeing who needs more support in groups such as noticing when
someone needs to take a 5-minute break or needs to process. She remains present when I
process with residents following groups and helps process with peers when needed. She is able
to notice when a resident needs their goal changed as they have either completed their previous
goal or need to work on something else.

Comments:
If I could give her a 6 in all of these categories I would. She has been very proficient at getting
documentation done in timely manner. She provides detailed comments or bx observed in
groups and does well to fully explain the resident’s participation in group.

She has done well at communicating to the RT team as well as the whole building as she is
planning a carnival for the whole building. She has not been able to attend an intensive staffing
meeting due to none of my residents on my caseload requiring them.

Comments:

She is aware of our outing budget and helps RT plan outing that fit this budget. She created the
schedule for the carnival and has been able to efficiently communicate with the whole building.

Comments:
She has not had much experience shadowing our department supervisor who is in charge of most
of these tasks due to the recent cyber-attack. She has done well to practice under the guidelines
of our theories.

Comments:
She was kind enough to speak in our internship fair video about her experience and expectations
at CHYC. She has done well reaching out to URTA to make sure she is able to virtually attend a
meeting next month.

Comments:
Overall, I have been very impressed with her willingness to learn. She shows up ready to learn
each day and is willing to take on challenges head on. She has accelerated the process of
observation and has begun to take over running groups under RT supervision. She mentioned she
does best by doing so as a supervisor I’m am trying to make sure she has as much opportunity to
get involved in the process and so far, she has done great. The only reason I gave her a 4 on using
open computer is because she will be working on her assignments such as case study, project, or
event on her own computer. She does very well with time management and gets everything done
on time and with detail.

Part Three: Comments


1. In what areas is the student-intern showing areas of growth and continued success?

She has shown strong growth with facilitating groups. We decided to co-lead a group
together, but she was on a roll and doing a great job, so I allowed her to take the larger
chunk of the group. She has taken these opportunities and ran with them, showing she has
the awareness, professionalism, and competencies required to run groups with this
population. Her documentation is also very detailed and precise as well.

2. In what areas is the student-intern not meeting competencies or needing further


development?
Running with processing discussions. She has just begun to ask more processing questions
but I want her to be thinking of follow up questions to residents comments and being able
to roll with the discussion that is happening, even if it’s not the planned discussion. I have
full confidence that with more practice she will be able to conduct longer more in-depth
processing discussions.
