Subject: Claim 2020102900009.R1 under Policy H0427032 by Phaneendra Kumar Patnala, Member ID H0427032-1-0 for treatment in NRI MEDICAL COLLEGE and
GENERAL HOSPITAL from 27/09/2020 to 02/10/2020

This is in reference to Claim documents received by us on 20 Nov 2020 and our earlier query letter dated 21/11/2020 sent to you.

We observe that following information/documents requested via the above query letter(s) are still not submitted.
Discharge summary.
Final bill.
Indoor case papers/ TRP charting/ SPO2 charting.
Passport copy ( front & back) with recent travel details. In case Passport of non availability of passport, provide self declaration stating the same.
6. Documentary proof stating whether NRI Medical college and General Hospital, Chinakakani is designated COVID hospital or not as certified by the
Government authority/ Municipal corporation.
We once again request you to submit the required information/documents within 7 days from the date of receipt of this letter, so as to enable us to conclude the claim at the earliest.
