You are requested to indicate the lowest price in Sri Lankan Rupees that could be afforded.
A Certified copy of the National Medicines Regulation Authority (NMRA – Sri Lanka) Registration Certificate, Certificate of Analysis
(COA) should be forwarded along with the quotation.

In addition, the following information also should be indicated in your quotation.

1. Delivery Date
2. Availability of Ex-Stocks (Ex- Stocks are expected to be supplied within two (02) working days after placing the order.
3. Delivery Schedule / date (in case if Ex- Stocks are not available)
4. Shelf Life of the item (Date of Manufacture and Date of Expiry)
5. Related Tender Number, SR number of the item and description of item.
6. Contact details of the bidder (Contact Person, TP Nos, (Land/mobile), Fax, e-mail)

Your quotation / quotations shall be placed in the relevant tender box; which is placed in the Internal Audit Department of the Head Office of
the State Pharmaceuticals Corporation. prior to closing date and time indicated above. (Any late delivered / submitted bids will be rejected
without accepting)

Tender Samples, with clear indication of the Tender No, shall be handed over to the Administration Department on or before the closing of
tender and acknowledgement of such samples should be attached to the quotation (bid).

SPC reserves the right to call samples from bidders who have not submitted samples before closing of the tender in cases of none of the
bidders had submitted samples, evaluators are with the opinion of calling samples from bidders who have not submitted samples and / or the
Procurement committee is with the opinion of calling of samples from the bidders who have not submitted samples will help them to make a
cost effective decision.

1. Please indicate the current market whole sale and Retail prices in your quotation (bid)
2. Your Quotation (bid) shall be submitted in one original and one duplicate, sealed separately and marked as “ORIGINAL” and
“DUPLICATE” respectively. Both envelopes shall be enclosed in one envelop and deposited in the tender box kept in the Internal
Audit Department.
3. In case if the supplier / manufacturer is not registered with NMRA – Sri Lanka, please provide following details together with the bid
(Quotation)
i) GMP certificate
ii) Certificate of Pharmaceutical Products
iii) Manufacturer’s information letter
iv) Certificate of Analysis of the Product
