ADOPTION REQUIREMENTS CHECKLIST

A. ADOPTIVE PARENTS

__ 10. Home Study Report from licensed Adoption Agency in
their place of residence, if foreigners.

B. ADOPTIVE CHILDREN

__ 3. Medical Certificate
__ 4. Psychological Evaluation
__ 5. Degree of Abandonment/Deed of Voluntary Commitment/Affidavit of Consent
__ 6. Death Certificate of mother/father, if applicable
__ 7. Death Certificate of father, if applicable

CHECKLIST OF REQUIREMENTS FOR ADOPTION

1. Birth Certificate (NSO) of child/children to be adopted


2. Birth Certificate (NSO) of prospective adopter/s
3. Marriage Contract (NSO) of petitioner/s or prospective
adopter/s , if applicable
4. Affidavit of Consent of the natural parents of the child/
children to be adopted
5. Affidavit of Consent of child/children (10 years and above)
to be adopted
6. Affidavit of Consent of child/children (10 years and above) of the petitioners
7. Affidavit of Support
8. Joint Affidavit of Three Disinterested Persons as to the (Good Moral Character) aspect of the petitioner/s
in the locality/community where they all reside
9. Documents to prove the Financial Capacity of the petitioner/s.
a. Transfer Certificate of Title (TCT) Certified by Registry of Deeds having jurisdiction over the place
where the properties are located to real properties (land) with corresponding certified copies of Tax
Declarations for land and buildings, preferably with appraised value/s by licensed appraiser.

b. Bank Certification of bank deposits


c. Motor Vehicle Certificate of Registration/Official Receipt (CR/OR)
d. Insurance Policy of petitioner/s
e. Certificate of Employment (if employed) from employer with statement on the number of years of
having worked therein, and current salary (if secured from abroad, Certification of Employment must be
authenticated by Philippine Consular Office or Embassy thereat).
f. Business permit, DTI Business Name, BIR Certificate of Registration as business entity
g. Individual Income Tax Return (ITR) for the current year, preferably audited Financial Statements
(Assets and Liabilities)
10. To prove that the petitioner/s is/are psychologically and physically fit to adopt:
a. Psychological examination of petitioner/s by a psychologist
b. Medical Certificate of petitioner/s
c. Medical Certificate of the child/children to adopted
11. Clearances of petitioner/s-
a. Barangay Clearance
b. Police Clearance
c. Prosecutor (Department of Justice) Clearance
d. Office of the Clerk of Court-Municipal/Metropolitan Trial Court (OCC-MTC-MTCC)
e. Office of the Clerk of Court-Regional Trial Court (OCC-RTC)
12. Pictures of the petitioner/s and child/children to be adopted
13. Legal Capacity to adopt issued by the applicable embassy of the petitioner/s (if foreigner)
