AFFIDAVIT OF NO EXISTING SCHOLARSHIP GRANTS

I, RUSSEL SILVA RAMOS, of legal age, Filipino citizen, single, and a

resident of and with postal address at 33 Tanqui Gabaldon, San Fernando
City, La Union after having been sworn to oath in accordance with law
hereby avers the following to wit:

That I am a graduate of Lorma Colleges with a degree of Bachelor of

Science in Medical Technology;

That I intend to pursue my medical studies at the University of

Northern Philippines this coming school year, for which I will be in my first
year of study;

That I am not a recipient of any scholarship grant/s from any

institution, government or private anent my forthcoming medical studies at
the University of Northern Philippines;

That I execute this affidavit in compliance with the school’s admission

requirement for my medical studies;

That I attest to the truthfulness and veracity of the foregoing facts and
statements based on my own personal knowledge in support of the
foregoing purpose and for all other legal intents and purpose/s it may
serve.
