AFFIDAVIT

I, Bikramjit, S/o of Sh. Tilak Raj resident of V.PO. Hardo Khanpur, HOSHIARPUR, do
hereby affirm and declare as under:-

1) That I, The Above named Deponent is a sleeping partner in the firm M/S Shivam
Medical Store situated at Opposite Balbhadar Vatika, Jodhamal Road, District
Hoshiarpur.

2) That I have not made any sort of investment in the above named firm, whether in
cash or kind.

3) That I am not entitled to any profit or loss in the firm or any assests or liabilities of
the same.

4) That I, the above named deponent, have dissolved my earlier partnership with MS.
Shalini Sharma(Redg. No-M28891) under the firm name of M/s Khushi Medical store
situated at Subhash Nagar, Dagana Road, Hoshiarpur and have absolved all my legal
formalities.

5) That this affidavit has been executed by me without any pressure or undue influence
rather it is my sweet will and consent.
