To PLDT:

We recognize that we, as customer, are responsible to provide adequate grounding. We also acknowledge that PLDT’s continued
service is fully dependent on a standard grounding source as it protects all equipment installed in customer premises including all
PLDT equipment and peripherals. With the foregoing, we are aware that PLDT maintenance SLA will be affected and also related
rebates will not be applicable until such time that adequate grounding is in place.

To ensure a more reliable service, we hereby commit to address and deliver the required grounding within one (1) month upon
signing of this document.
