This letter serves as a formal request for a refund of P70.00–the total paid to the University, for
student Identity Card replacement on October 26, 2020.

The reason I am requesting a refund is because initially I was swapping the student card and there
was no need to have payed for the services.

Please refund this balance by cash in hand,that was used to make the initial payment by Wednesday
28, 2020.

I attach the receipt in this letter as proof of purchase.
