AFFIDAVIT OF LOSS

I, JENILYN MERCADO, of legal age, single, Filipino and a resident of

218 Remy St. Pasay City, after having been duly sworn to in accordance with
law, hereby depose and state:

That I am a holder of a POSTAL ID issued on January 2, 2020.

That said POSTAL ID which was kept in my wallet, was discovered missing
last July 2020 when I was looking for it;

That despite diligent efforts to look for and locate the same proved futile;
hence, it is presumed as lost and deemed beyond recovery;

That I am executing this affidavit in order to attest to the truth of the

above and to justify the issuance in my favor of a new POSTAL ID, in lieu of the
lost one and for all legal intents and purposes.
