KNOW ALL MEN BY THESE PRESENTS:

I,ROME E, of legal age, married, Filipino citizen and a resident of of Santa Rosa, Laguna herein
referred to as the FIRST PARTY;

and

GEM, of legal age, married, Filipino citizen and a resident of, Calamba City, Laguna herein
referred to as the SECOND PARTY;

WITNESETH:

WHEREAS, the parties are the biological parents of minor (the CHILD) born
on ______________;

WHEREAS, the parties desire to provide for a complete settlement of the issues pertaining to
the custody, visitorial rights, support and maintenance of the child as well as to the legal wife;

WHEREAS, each party acknowledges his or her personal obligations as parent of the child and,
by these presents, each hereby undertakes to render the performance of these obligations to
the family and comply with his or her duties as a parent;

NOW, THEREFORE, for and in consideration of the promises and dispositions made in this
agreement, the parties hereto have agreed as follows:

1. The PARTIES are legally married and the biological parents of FRANCCA, 3 years old,
who is now under the care and custody of the SECOND PARTY;

2. The FIRST PARTY, being the father of their only child and legal husband of the
SECOND PARTY, obligates himself to provide for the financial support for the family;

3. The FIRST PARTY immediately effective upon the signing of this agreement, without
need of demand, shall pay to the mother the sum of SIXTEEN THOUSAND PESOS
(16,000) monthly, payable in half every 10th day and 25th day of the month for the
support and maintenance of the child during the period of his minority, or until further
ordered by any Court of competent jurisdiction in the Philippines.

4. When the FIRST PARTY loses his job, the support aforementioned herein will
automatically be stopped. However, in case his work will resume, support also resumes
but notice of job termination/job loss will be presented to the SECOND PARTY;

5. When the finances of the FIRST PARTY improve in the future, the aforementioned
agreed support will be adjusted accordingly depending on the child’s need and of the
family;

6. The parties agreed that the custody of their child, shall remain with the SECOND
PARTY, with visitorial rights from the FIRST PARTY. The FIRST PARTY may visit the
child but is not allowed to enter the dwelling or he may bring his daughter to his
hometown with the prior permission from the SECOND PARTY;

7. The parties herby bind themselves to faithfully abide by the terms of this agreement.
Should either one of the parties fail to comply with the terms and conditions of this
Agreement, the aggrieved party may seek judicial relief against the erring party and
apply with the proper court for a writ of execution against said erring party to enforce his
or her obligations imposed in this Agreement. The offending party shall pay for the cost
of litigation, attorney’s fees, other expenses, and interest incurred in such application for
a writ of execution.
