Philippines, do hereby depose and say that:

1. I am the CCTV Operator of STARLITE MARINE INDUSTRIAL

INCORPORATED, a corporation duly organized and existing under the
laws of the Philippines;

2. That an unremitted terminal fee stub worth Php20,000.00 was

under my possession however I failed to account it;

3. I promised to pay the above-mentioned amount on or before

March 5, 2020;

4. That I execute this affidavit voluntarily to attest to the truth of

the foregoing statements and for all legal purposes it may serve.

2. There is an unremitted Terminal Fee Stubs worth P2,000,000.00

that is under my possession which I cannot account;

3. That I promise the above-mentioned company to pay the said

amount within EIGHT (8) Years with a monthly payment
amounting to TWENTY THOUSAND EIGHT HUNDRED THIRTY-
THREE (Php20,833) starting April 30, 2020 until April 30, 2028;

4. That I execute this affidavit voluntarily to attest to the truth of
