AFFIDAVIT OF CONSENT
We,

Loon, Bohol, Philippines, after having been duly sworn in accordance with
law, hereby depose and state:

1. That we are the heirs of PORFERIO CARLON, who is the owner of a

parcel of land situated in Looc, Cabilao, Loon, Bohol, Philippines,
containing an area of ________________square meters covered
under Tax Declaration No. _____________;

2. That CLAUDIA NESSA CATAYAS will intend to build a house on the said
area and we hereby signify our written consent, freely, willingly and
voluntarily to allow him to build a house on a portion of the said parcel
of land and, consequently, have an electric connection thereon;

3. That we also authorized them to process him all the permits and
requirements needed for the said building;

4. That we are executing this affidavit to attest the truth of the foregoing
facts as to our undertaking to give consent to them and to support
their application for a Building Permit and Electrification Permit also.
