The Petitioner, through the undersigned counsel, unto this

A-1 - Signature of the Petitioner
PURPOSE: To prove that the Petitioner, duly verified and filed the
Petition before this Honorable Court alleging, among others,
the entries in Certificate of Live Birth of the Petitioner which
was issued by the NSO are erroneous and contrary to what
she has been using and known.
B - The Order of this Honorable Court dated
19 February 2018 giving due course to
this Petition, found on page 11 of the
Record

PURPOSE: To prove that this Honorable Court has determined that

the Petition is sufficient in form and substance, and has
ordered the conduct of the hearing of this case, and for such
other related purposes.

C - Letter of Authority

PURPOSE: To prove that the Office of the Solicitor General hereby

authorized the Office of the City of Prosecutor of Tagbilaran
City to appear in their behalf.

D - Notice of Appearance

PURPOSE: To prove that the Solicitor General through the Office of

the City Prosecutor entered its appearance as counsel for
the Republic of the Philippines in the above-entitled case,
and cause all notices of hearings, orders, resolutions,
decisions and other processes to be served upon them.

PURPOSE: To prove that the Petition of Nicefora Daleon Niluag
a.k.a Nicepura Dalion Linuag for change of name/and or
correction of entries in the Certificate of Live Birth has been
published in The Bohol Tribune, a newspaper of general and
national circulation on March 26, 2017, April 2, 2017 and
April 9, 2017.

F - Certificate of Live Birth

PURPOSE: To prove that a certain person named Nicefora Niluag

a.k.a Nicepura Linuag, born on February 26, 1957 at Booy,
Tagbilaran City is registered in the Civil Registry of
Tagbilaran City.
G - Certificate of Baptism
PURPOSE: To prove that the true and correct family name of
Nicefora is Niluag, that her birthday is celebrated every
February 25, 1957 and that the family name of her mother is
Daleon.

H - Marriage Contract
PURPOSE: To prove that the true and correct family name of
Nicefora is Niluag and that the family name of her mother is
Daleon.

I - Elementary School Permanent

Cumulative Record
PURPOSE: To prove that the petitioner continuously used the name
Nicefora Daleon Niluag since her childhood.

J - Medical Certificate
PURPOSE: To prove that the petitioner is grossly female.

K - Police Clearance
L - NBI Clearance
PURPOSE: To prove that the petition is not filed to hide the identity of
the petitioner from any lawful authority, neither it is done for
the purpose of evading punishment for a crime.
PRAYER

WHEREFORE, premises considered, it is most respectfully

prayed that the above listed documentary exhibits be admitted as
evidence for the Petitioner for the purposes these are offered, and
that these shall form part of the oral testimony of the witnesses for the
Petitioner.

Petitioner hereby manifests that after the admission of the said

exhibits he respectfully rests his case.

Other reliefs and remedies as may be deemed just and

equitable under the premises are likewise prayed for.
