JOINT-AFFIDAVIT OF DESISTANCE

We, JOEY R. ANTONIO, of legal age, married, Filipino citizen, resident of and
with postal address at Brgy. Lacong, San Gabriel, La Union and WINCESLAO T.
ESPADA, also of legal age, married, Filipino citizen, and resident of and with postal
address at Brgy. Lacong, San Gabriel, La Union after having been duly sworn to in
accordance with law hereby depose and state that:

We are the private complainants in the criminal case docketed as Criminal Case
Nos. 3919 and 3920, respectively for Falsification of Public Document by a Public
Officer/Employee pursuant to Article 171 (1) of the Revised Penal Code filed against
JOHN MARK SACQUIATEN before the 1ST Municipal Circuit Trial Court of San
Gabriel-San Juan, La Union but subsequently ordered dismissed without prejudice by
the court upon motion of the Public Prosecutor due to LACK OF JURISDICTION by
the said court; and now awaiting final resolution by the Office of the Provincial
Prosecutor, San Fernando City, La Union for its proper and final disposition;

We, the private complainants and John Mark Sacquiaten had already settled our
differences to the satisfaction of both parties and finally manifest that we are no longer
interested and hereby desist, in pursuing the criminal complaint against John Mark
Sacquiaten;

Moreover, we are now requesting the Honorable Provincial Prosecutor to have

the said criminal complaint be DISMISSED against JOHN MARK SACQUIATEN;

We are executing this affidavit freely and voluntarily, under our express will and
direction and that the same was read in language known and understood by both of us
and we are aware of the consequences of our execution of this affidavit which we both
understand before we affixed our signatures.
