AFFIDAVIT OF LOSS
(ORIGINAL CERTIFICATE OF TITLE)

I, MARIA LIZA PAPARON PADILLO, Filipino, of legal age, married, and a

resident of F. Torralba St. Cogon District, Bohol, Philippines, after having duly
sworn to in accordance with law, do hereby depose and say:

1. That I am the owner of a parcel of land situated at Cogon District,

Tagbilaran City, Philippines, designated as Lot No. 1507-3-C-4-B,
containing an area of One Hundred Eighteen (118) Square Meters,
covered by Transfer Certificate of Title No. T-53981 issued by the Register
of Deeds for the Tagbilaran City by virtue of a Deed of Absolute Sale
notarized by Atty. Reynard D. Namocatcat;

2. That my father assigned me to safe keep the certificate of title of the said
parcel of land;

3. That on June, 2008, a month before the death of my father, I tried to

retrieve the certificate of title from the cabinet where I placed my important
documents. Despite earnest effort to locate the folder where I placed the
said certificate of title, I could not find the same.

4. That ever prior to the execution of this affidavit, I still took pain in finding
the said original owner’s copy of OCT No. 55154, yet despite earnest
effort on my part to recover, retrieve and/or find said OCT, the same had
not been recovered, retrieve and/or found; thus I have strong reason to
believe that the same is now lost for all legal intents and purposes;

5. That no petition in Court was filed by me and/or any other interested

person for the issuance of a new owner’s duplicate copy of the said land
titles and that the same has not been sold, pledged, mortgaged or
otherwise delivered to any person or entity to guarantee any obligation or
for whatever purpose;

6. That I am executing this affidavit of loss to attest to the veracity of the

above-stated facts, for the purpose of the issuance of a new Original
Certificate of Title or any other legal purpose which this affidavit may
serve.

1. That I am the nephew of SPOUSES ARTEMIO RODRIGUEZ and

FRANCISCA DOMINGUEZ RODRIGUEZ, the owner of a parcel of land
situated in Cogon District, Bohol, Philippines, designated as Lot No. 1507-
3-C-4-B, containing an area of One Hundred Eighteen (118) Square
Meters, covered by Transfer Certificate of Title No. T-53981 issued by the
Register of Deeds for the Tagbilaran City;

2. That a portion of the above-mentioned parcel of land designated as Lot

1507-3-C-4-B-1 containing an area of Twenty-Eight (28) square meters
was bought by me, by virtue of a Deed of Absolute Sale notarized by Atty.
Reynard D. Namocatcat;

3. That on June, 2009, when I am starting to process for the transfer of the
title, the said title was lost and its whereabouts cannot be ascertained, I
tried to retrieve the certificate of title from the cabinet where I placed my
important documents. Despite earnest effort to locate the folder where I
placed the said certificate of title, I could not find the same.

4. That ever prior to the execution of this affidavit, I still took pain in finding
the said original owner’s copy of Transfer Certificate of Title No. T-53981,
yet despite earnest effort on my part to recover, retrieve and/or find said
TCT, the same had not been recovered, retrieve and/or found; thus I have
strong reason to believe that the same is now lost for all legal intents and
purposes;

5. That no petition in Court was filed by me and/or any other interested

person for the issuance of a new owner’s duplicate copy of the said land
titles and that the same has not been sold, pledged, mortgaged or
otherwise delivered to any person or entity to guarantee any obligation or
for whatever purpose;

6. That I am executing this affidavit of loss to attest to the veracity of the

above-stated facts, for the purpose of the issuance of a new Original
Certificate of Title or any other legal purpose which this affidavit may
serve.
