But nevertheless, this is not comparable to a reshus harabim, for a reshus harabim is
something that is not gone from the worlds, and in our time in the large cities where
great masses congregate, there is certainly a reshus harabim. But grinding medicines
continually diminishes from the world with the advancement of medicine and
pharmacies, for in every place where they are found, individuals do not engage in
their own remedies.

And one can make this reasoning into a partial component [for permitting healing
on Shabbos], such as where poskim are [already] in disagreement, this reasoning
can be applied to permit. Likewise, there are doubtful [kinds of] people who, even
if they are in great pain, can't [seem to] decide for themselves if [they are in pain
enough to be in the halachic category of] their entire body being weak from this
[i.e. they feel so bad that it interferes with their normal functioning], or if they
have not [quite] reached the status of sick in their entire body. [These people]
may rely on the heter of healing nowadays when the concern of grinding
medicine is not so applicable, as mentioned above. But for someone who is just
[moderately] uncomfortable, who everyone otherwise says [falls under the]
prohibition of healing [on Shabbos], it should not be permitted even nowadays.
