1. What are the possible causes why the patient is having this one. 

 The cause can be the sinus infection of the client, this infection causes the
rupture of tiny vessels that causes bleeding on her nose.

  2. Formulate your plan of care for this patient.

 Position her on upright or leaning forward position to prevent her from swallowing
the blood and to prevent aspiration
 Apply direct pressure by pinching the nose against the middle septum 5-10
times.
 Tell her to breathe through her mouth while she holds firm pressure on the soft
flesh of her nose for at least 10 minutes. 
 After bleeding is controlled, reassess the patient and provide oral care.
