1. Introduction
The Supplier shall pro-actively manage, assure and control Occupational Health and Safety for all
personnel working on or in association with the Services provided under the Agreement.
Attention is required to be focused on the highest risks to Occupational Health and Safety well in
advance of potential manifestation of these, and treatment is applied in a timely and as-required
manner, such that the risks are eliminated, prevented and/or adequately controlled.

The scope of coverage of Occupational Health and Safety shall be operationally focused and
targeted to ensure the achievement of zero fatalities and zero injuries (major or minor) to
personnel associated with the projects, services, products, and material being delivered to IO. As
part of this, positive performance indicators and targets must be employed, focusing the attention
on the capability of the personnel, teams, subcontractor, suppliers, etc. to be able to properly
manage Occupational Health and Safety, and not expose their personnel, employees, contractors,
or the general public to any Occupational Health and Safety risk.
2. Purpose
It is the purpose of this document to outline the mandatory minimum requirements for Suplier’s
to manage the Occupational Health and Safety risks to any personnel working on or in association
with the Services under this Agreement, and the monthly reporting and presentation of how they
are managing these Occupational Health and Safety risks, and performance following the
Governance Process.
3. General Requirements
The Supplier’s local entity shall have a documented Occupational Health and Safety management
system to ensure effective planning, management and control of Occupational Health and Safety.
This Occupational Health and Safety system shall satisfy the requirements of both the OHSAS
18001:2007 and OHSAS 18002:2007 or later revision as available from the international
standards organisation.
Such certification must at a minimum cover the Scopes as follows:
a. Implementation, installation, testing and commissioning of telecommunications equipment,
and supporting site materials and infrastructure.
b. Operation and maintenance, after sales service support of telecommunications equipment,
and supporting site material and infrastructure.
c. Project management of the implementation, installation, testing and commissioning of
telecommunications equipment, and supporting site materials and infrastructure.
d. Management of operation and maintenance, after sales service support of telecommunication
equipment, and supporting site material and infrastructure.
e. In addition to this requirement, the Supplier must also maintain key personnel within its
national organisation and dedicated to any services supporting project or other activities
provided to IO, certified as auditors for this management system and to these standards.

4. Occupational Health and Safety Management Policy Statement


4.1 IO is committed to the preservation of health and assurance of safety in the work place for all
its own and associated 3rd party personnel including employees, contractors (long-term,
short-term, temporary), consultants, suppliers personnel, sub-contractors personnel, service

provider personnel and member of the public, and will create a culture throughout the
workplace and its operation to ensure that within internal and 3rd party operations, all
subsidiaries internal and related 3rd party operations and with reference to Indonesian
national standards, codes of practice, guidelines, regulations, and those of any country of
operation, taking the highest standards as the default requirement:
a. Engagement and use of safe working practices and best practices for health preservation
and sustainment
b. Identification of hazards and the subsequent reduction of any personal risks posed to
acceptable levels
c. Maintain compliance to the relevant legal requirements, guidelines, recommendations
and code of practices
d. Implementation of international best practices for occupational health and safety across
entire operational portfolio
e. Implementation of improvement programs

4.2 All personnel in the company will work together to implement relevant elements of this
policy and every person is responsible for Occupational health and safety in the work place,
and as such will:
a. Highlight and correct unsafe work practices they may come across
b. Support open dialog and consultation between management, employees and their
nominated representatives, contractors and customers on all aspects of the company’s
occupational health and safety program, and
c. Comply with relevant legislative requirements, regulation, order(s), codes of practices,
guidelines and international best practices, along with the company’s own internal
procedures and any additional customer requirements for safe working practices.
d. Hazard/ risk management:
i. Planning: occupational health and safety forms an integral part of the company’s
business objectives, and sufficient resources are allocated to manage all aspects of
occupational health and safety in the organisation
ii. Leadership: managers are equipped with the training and resources to ensure
occupational health and safety issues are properly dealt with in their areas of
responsibility.
iii. Training: all employees are provided with the appropriate levels of training.
Competency development and on-going updates to enable them to perform their
duties in accordance with established procedures.
iv. Performance review: senior management reviews are undertaken at least annually,
aimed at monitoring the performance of the occupational health and safety
management system against the afore-mentioned planning objectives.
v. Personal protective equipment (PPE): where all other methods of improving safety
have been exhausted, and personal protective equipment is needed to make work
activities safer, the company shall provide all necessary PPE for workers and ensure
that all 3rd party subsidiaries, suppliers, subcontractors and service provider
organisation do the same
vi. Incident management: all incidents will be accurately reported, recorded and
managed to minimise impacts and prevent their re-occurrence

vii. Welfare: employee participation in health and well-being programs will be
encouraged. Where required, we will provide employees and their families with
ongoing assistance, counselling and support
viii. Continuous improvement: IO is committed to improving its systems and will
implement corrective actions, undertake external benchmarking and participate in
the sharing of information to ensure the on-going effectiveness of the occupational
health and safety management system.
ix. This policy statement will be reviewed and revised as necessary on periodic basis.

5. Occupational Health and Safety management applied to Projects, Operations and


Implementation Services
The Supplier, as part of the implementation of Occupational Health and Safety within its
organisation for Services and Operations, and the associated Projects and Services being
delivered to IO, is required to actively apply the Occupational Health and Safety Management
methodology, processes and procedures documented by its management system mentioned
above. Specifically, the following points need to be covered as a minimum (but should be
extended where required and practicable):
a. Create as required (if not already existing), and submit Occupational Health and Safety
Management policy that covers the Services, based on the principles and in alignment with
OHSAS 18001:2007 and OHSAS 18002:2007.
b. Supplier shall create and submit to IO an Occupational Health and Safety Management plan
at the Service Commencement Date, based on the principles and in alignment with OHSAS
18001:2007 and OHSAS 18002:2007.
c. Maintain an active Occupational Health and Safety risk register that identified all
Occupational Health and Safety risks associated with the delivery of contracted projects,
services and deliverables. Occupational Health and Safety register should record and
maintain a rating of the potential impact / consequences of not mitigating or managing the
Occupational Health and Safety should it manifest, and the likelihood of its manifestation, and
based on these values, the overall unmitigated risk rating. The Risk Register shall also record
all identified mitigation actions inclusive of whether the Occupational Health and Safety may
be eliminated, or if mitigation actions are possible and necessary, what these mitigation
actions might be, whether the Occupational Health and Safety could be transferred, and what
the revised likelihood of the Occupational Health and Safety and therefore overall mitigated
Occupational Health and Safety rating becomes.
d. Supplier shall perform risk assessment and evaluation across all identified risks on a basis
agreed within the Occupational Health and Safety Management plan, relating to the Services
under the Agreement under the forums in the Governance Model for IO to review the
potential likelihood and impact / consequences of identified risks.
e. IO on a weekly or as required basis, apply the necessary treatment of the Occupational Health
and Safety risks per item 2 above.
f. Perform an Occupational Health and Safety identification activity, identifying new
Occupational Health and Safety risks and dependencies that may potentially affect all project
related, operational and financial aspects of the delivery of services, products and materials
to IO on a frequency outlined in the Occupational Health and Safety Management plan.

g. Reporting of the current Occupational Health and Safety risks including newly identified
Occupational Health and Safety risks for the Services to IO on a monthly basis at the forums
under Governance Process, utilising a summary report showing all identified Occupational
Health and Safety risks and their rating on a likelihood vs consequence chart.
h. Reporting of the current performance of the Services for Occupational Health and Safety to
IO on a monthly basis including but not limited to:
i. Supplier Near- misses, minor and major injuries to personnel, temporary and
permanent disabilities suffered by personnel, fatalities.
ii. Property damage suffered
i. Supplier shall pro-actively manage, mitigate and control Occupational Health and Safety risks
such that attention is focused on the highest Occupational Health and Safety risks well in
advance of potential manifestation of these.
j. Maintain a current, up to date, library of job safety analysis conducted for each and all types
of work to be performed for all types of sites. The library should include the generic job safety
analysis, including the rating of the risks and agreed elimination or mitigation methodology
for each, generic and some up to date actual site-specific cases for each.
k. Maintain a current, up to date, library of safe working method statements prepared for each
and all types of work to be performed for all types of sites. The safe working, method
statement should be based in the afore mentioned job safety analysis already performed as
applicable. The library should include the safe working method statements, and some up to
date data actual site specific cases for each.
l. Implement a formal permit to work system for all personnel working on or in association
with the Services being delivered to IO or supporting such, including internal personnel
(employees, long term contractors, short term contractors, etc) and Subcontractor personnel
(employees, long term contractors, short term contractors, etc).
i. Such a permit to work system shall be supported by an information technology system
and database, covering all the requirements to maintain current up to date information
relating to the technical and safety certification, competence, experience and capability
of all registered personnel to perform their assigned work.
ii. Supplier shall provide access to permit to work system to IO and any of its designated
representatives including internal employees and third party provided consultants
supporting IO project and operations.
iii. Supplier shall establish as necessary multi-part on-disclosure agreements between IO,
itself its suppliers and subcontractors and service providers, and any designated third
parties of IO, to ensure there is no impediment to the provision of such access and that
confidentiality the personnel shall be ensured by all parties.

6. Competency Required By IO To Manage Occupational Health And Safety


Supplier shall maintain an appropriate number of component (trained, certified, experienced)
personnel to manage Occupational Health and Safety across all aspects, in particular (but not
limited to the below)
a. Minimum 3 senior management personnel within the Territory fully trained and certified in
international Occupational Health and Safety management standard NEBOSH International

general certificate for Occupational Health and Safety management. Such personnel may
include the human resources manager, and the designated Occupational Health and Safety
manager of the company, however cannot be the same person in a dual role
b. Minimum 2 senior management personnel within the Territory fully trained and certified in
local Occupational Health and Safety management standard “Ahli K3 umum”. Such personnel
may include the human resource manager, and the designated Occupational Health and
Safety manager of the company, however cannot be the same person in a dual role
c. Minimum of 2 senior management personnel within the Territory fully trained and certified
in International Occupational Health and Safety management auditing standard OHSAS1001
lead auditor. Such personnel may include the human resources manager and designated
Occupational Health and Safety manager to the company, however cannot be the same person
in dual role
d. Minimum of 2 senior management personnel within the Territory fully trained and certified
in International Occupational Health and Safety management auditing standard SMK3. Such
personnel may include the human resources manager and designated Occupational Health
and Safety manager to the company, however cannot be the same person in dual role
e. Minimum of 2 personnel within the Territory dedicated to working and supporting the
project or service, fully trained and certified in International Occupational Health and Safety
management auditing standard NEBOSH International general certificate for Occupational
Health and Safety management. Such personnel may include the human resources manager,
and the designated Occupational Health and Safety manager of the company (cannot be the
same person in a dual role)
f. Minimum of 2 personnel within the Territory and dedicated to working and supporting the
project or service, fully trained and certified in International Occupational Health and Safety
management auditing standard OHSAS18001 Auditor. Such personnel may include the
human resources manager, and the designated Occupational Health and Safety manager of
the company (cannot be the same person in a dual role)
g. Minimum of 1 personnel within he local entity and dedicated to working and supporting the
project or service, entity fully trained and certified in International Occupational Health and
Safety management standard “Ahli K3 umum” and “ahli K3 listrik”. Such personnel may
include the human resources manager, and the designated Occupational Health and Safety
manager of the company (cannot be the same person in a dual role)
h. Minimum of 1 person responsible within the Territory dedicated to working on and
supporting the project or service in related to perform the Services under the Agreement,
fully trained and certified in International Occupational Health and Safety management
auditing standard OHSAS18001 auditor
i. All respective project manager, coordinators, quality managers, quality engineers, safety
inspectors, supervisors, subcontractors project managers, subcontractor team leaders,
subcontractor supervisor, as a minimum must be trained and certified, wherever necessary
to its work, in (as relevant)
i. SMK3
ii. Ahli K3 Listrik
iii. OHSAS18001 general

ix.Correct usage of and care for personal protection equipment
x.All personnel working on IO sites must be trained in first aid and have a first aid kits
available at the sites at all times that is in accordance with the IO standard requirements
for first aid kits (see project safety instruction for first aid kit available to them on site at
all times that is safety instruction for first aid kits for project and operation work on
site)
j. All personnel performing electrical works or installing equipment or materials that will come
into any contact with electrical system or connect to electrical system must be trained and
certified in Indonesian government and LPJK authorised program leading to a certified LPJK
Teknisi licence, and covering (but not limited to) all relevant modules as applicable to the
scope of electrical works and/or equipment being installed, repaired, maintained.
k. All personnel perform work at height e.g on antenna mounting structures such as self-
supporting towers, guyed masts, monopoles and/or rooftops, or any such location as defined
as working @ height by the Indonesian government and/or >=1.8 above the ground, shall be
trained and certified, in Indonesian government (Menakertrans) authorised program that is
also approved by IO, leading to certification for working at height including specific coverage
of telecommunication tower climbing.
l. It is important for the Supplier to note that such training does not need to include rope access
modules. The Supplier may choose to train its own personnel including those that may be
utilised on buildings, for painting antenna mounting structures, cleaning windows, abseiling,
roc climbing, etc. however this shall not be a substitute for personnel learning and being
certified in the climbing of antenna mounting structures from ground up.

7. Non-Exclusive List of Applicable Laws Pertaining to Occupational Health and Safety


Set forth below is a non-exclusive list of certain laws pertaining to health and safety which IO
considers are applicable to maintenance and operation of the sites
a. Undang-undang No 7 tahun 1981 -tentang pelaporan tenaga kerja di perusahaan

c. Undang-undang No 3 tahun 1992 -tentang jaminan social karyawan
