AFFIDAVIT OF LOSS

I, REBECCA B. EVANGELISTA, of legal age, Filipino, and

presently with residence and postal address at San Pablo, Laguna,
after having been sworn to in accordance with law do hereby depose
and say: That –

1. – I am the petitioner in LRC Case No. 16-605 pending

before the Regional Trial Court –Branch 206 – Muntinlupa City;

2. - Before I left for the United States to find work in the said
country, I left some of my public documents, which includes the
torrens title of the property, which is covered by T.C.T No. (168935)
071667 of the Registry of Muntinlupa with an area of 127 square
meters, more or less located at Barangay Bayanan, Muntinlupa,
which I was able to acquire when she was then single for safekeeping
with my niece, SAHLEE V. TRIVINO;

3. – According to my niece, she kept the said documents in one

plastic envelope in one of the cabinets in her room at here residence;

4. - When I came back home in October 2015, I asked the

whereabouts of the public documents I left her for safekeeping,
specially the torrens title;

5. – After asking the whereabouts of the said documents,

Sahlee v. Trivino, immediately looked for it in her residence. But
despite diligent efforts she was not able to locate the same;

6. – So, she executed an affidavit of loss over the subject

property which was duly annotated with the Register of Deeds of the
City of Muntinlupa;

7. I am executing this affidavit to attest to the truth of the above

facts for all legal intents and purposes that it may serve.

AFFIANT FURTHER SAYETH NAUGHT.
