PARENTAL CONSENT FORM FOR GSP@80: NOVEMBER VOICE OUT

This is to certify that I allow my daughter, ____________________ of ______________ Council,

______________ Region, to participate in the GSP @ 80: November Voice Out Event of the Girl Scouts of
the Philippines and Together We Rise Philippines which aims to address the Violence Against Women
and Girls (VAWG) that is happening in the country through raising awareness, encouraging and
empowering the girls to take part, and amplifying our voices using social media.

I understand that her participation in this online activity is voluntary and that the project team shall
ensure the safety of my daughter in this activity. I also allow the Together We Rise Philippines of the Girl
Scouts of the Philippines for full use of the images and video outputs of my daughter which will help
achieve the event's goal, which is to aid in eradicating VAWG. This includes, but not limited to, the right
to use them in the promotional materials and other contents of Together We Rise Philippines' page.

I have read, understood, and agreed with the above provision.
