1 Product Description
The formula is developed to meet the nutritional requirements of healthy and active infants aged 6 – 12 months. This formula is
manufactured from fresh Australian milk, vegetable oil blend, imported milk solids and fortified with minerals, vitamins and other essential
nutrients that support the growth and development of infants.
2 Product Application and Intended Use
This product is intended for infants aged 6 – 12 months, as a supplement to a normal infant’s diet and should not be their sole source of
nutrition. Toddlers over 12 months should be offered solid foods in addition to formula.
This product should not be offered to infants under the age of 6 months.
3 Declaration and Warranty
Blue Lake Dairy Group has implemented the following Food Safety Management Systems:
 State Registration through Dairy Authority of South Australia
 Department of Agriculture Export Control (Milk and Milk Products) Orders 2005
Blue Lake Dairy Group:
1) Certifies that this product complies with all relevant standards within the Australia New Zealand Food Standards Code. Where
applicable, and that where such certificate relies on third party audits, analysis, industry codes, or equivalence of international
standards to demonstrate compliance, that certificates are current and available;
2) Acknowledges that the Customer, and Supply Chain Customers of the Customer, will rely on the accuracy of the Product
Information for food quality, safety and labelling purposes;
3) Certifies that the accuracy of the Product Information is limited to the following degree: -
a. that the Product Information in relation to ingredients obtained from a third party relies in good faith on Product Information
provided by that third party;
b. that the information is, to the best of the supplier’s knowledge (having undertaken all reasonable verification procedures),
true and accurate in relation to all other substances and processes;
4) Agrees that all product it supplies to the Customer will conform with the Production Information unless otherwise agreed to in writing
and in advance by the Customer;
5) Will immediately inform the Customer (and confirm in writing as soon as possible) if supplier becomes aware of any error or
omission in the Product Information;
6) Will inform the Customer in writing and in advance of any change to the Product Information provided herein (including any changes
that result from new or modified processes) if and when the supplier becomes aware of such changes; and
7) Acknowledges that the Customer may provide the Product Information to –
a. regulatory agencies in relation to any matter raised by such agencies
b. courts and other legal tribunals for the purpose of any proceedings; and
c. to its related businesses and partners who are involved in the acquisition, use, sale or compliance of the Product, under this
same restriction as disclosure.

8) Acknowledges that, subject to the prior written agreement of the supplier and any restrictions nominated by the supplier in regard to
disclosure of confidential information, the Customer may provide the Production Information to its own customer subject to those
customers ensuring the information is not further disclosed.

5 Nutrients and Consumer Information Claims
5.1 Serve Size
To reconstitute the product put one scoop (4.3g) of powder per 30ml of water as per the feeding table below.

5.2 Preparation
1) Thoroughly wash bottle, teat and cap.
2) Boil the bottle, teat and cap for at least 5 minutes.
3) Cool the boiled water to 45°C, then pour the correct amount of water into the sterilised bottle.
4) Add the required number of levelled scoops of powder, using only the enclosed scoop. Level each scoop of powder with the lip of the can
or the back of a sterilised knife.
5) Close and shake the bottle until milk powder is completely dissolved.
6) Test temperature on wrist before feeding. Feed immediately. Throw away any unfinished feed.

Follow instructions exactly. Prepare bottles and teats as directed. Do not change proportions of powder except on medical advice.
Incorrect preparation can make your baby very ill.
6 Durability, Packaging and Supply Chain
6.1 Shelf Life
24 months at 15-25°C in the dark at relative humidity of <60%. Store in a tin tightly closed in a dust and pest free, cool and dry place.
Use within four weeks of opening. Close lid tightly after each use and store in a cool dry place. Use by date is on the base of the can. Do
not use if oil seal is missing or damaged. If correctly stored and made up in accordance with the directions on the label, no further vitamin
or mineral preparation are necessary.
